Hello,

I am planning to make home visits August 3-5. I will be calling you shortly to make an appointment to meet with you and your child. If you have your child's dental and health record, I can take those at that time. A copy of your child's birth certificate will be needed also.

I'm looking foward to meeting with you and your child.
