home visits

Greetings! I'll be calling you soon to make an appointment with you and your child for a home visit. I am planning to do these August 5-7. If you have your child's dental and physical form, I will take them at that time. Also needed is a copy of your child's birth certificate. The visits take about 20 minutes.
